Alfredo was the childhood friend of Giulietta. His picture was always on display in Ettore's household.
While Giulietta was courted by both Indiana Jones and Ernest Hemingway during World War I, Indy believed that the picture was of his unknown rival before he realized it was Hemingway. Ironically, Giulietta eventually decided to marry Alfredo, and did not choose among the two other men.
